Your cover image may be physically too large for Kobo to handle, so it gives up. Try reducing the image size of the image.
You can do this by importing a smaller image into your project in the Cover page of the Project Properties dialog, or (easier) setting the configuration option Cover image width to a smaller value so that Jutoh resizes it when the Epub is created. Try 1200, for example, or a smaller value if that still doesn't work.
