If you compile an OpenDocument (ODT) file with Jutoh and import it into an application such as LibreOffice, you can export it to a PDF, and Adobe Digital Editions will show headings with an outline level of 1 in the contents. This is similar to the logical contents or navigation map (NCX) in an Epub file.
However, when using different level 1 heading styles, for example "Heading 1" for most chapters and "Index Heading" for the index, the PDF contents hierarchy may display incorrectly within ADE, nesting headings erroneously.
You can can fix this by editing the configuration option Paragraph style substitutions and setting it to, for example, "Index Heading:Heading 1" in order to replace "Index Heading" with "Heading 1" in the compiled ODT file.
