On Kindle devices and Kindle Previewer, using the navigation map (logical contents) can have unexpected results, where (for example) navigating to a title page goes to the cover page. This might make you think there are two covers, when in fact two contents entries are pointing at the same page. This is a bug in Kindle.
The solution is to ensure that the navigation map references bookmarks (fragment ids) and not just the HTML file. One method is to use the advanced table of contents instead of letting Jutoh generate the table of contents. Use the command Format | Book | Build Table of Contents and let Jutoh scan the project sections, building a table of contents. You can always delete the table of contents section that Jutoh creates (or clear Create a contents page in the wizard); the important thing is that navigation map entries contain bookmarks, as seen in the Indexes/Contents page in the Project Properties dialog.
From Jutoh 2.63, if you haven't defined an automatic TOC, Jutoh will automatically add bookmarks at the top of each chapter for Kindle books, and link to these from the NavMap, thereby working around the problem.
