Sometimes Lulu rejects an ebook with errors like this:
ERROR: IOException: Unresolved external XML entity: 'http://www.w3.org/TR/xhtml11/DTD/xhtml11.dtd' in OPS/section-0001.html
ERROR: Unable to locate element with id 'auto_bookmark_table_of_contents_1' in OPS/toc.ncx
ERROR: IOException: Unresolved external XML entity: 'http://www.w3.org/TR/xhtml11/DTD/xhtml11.dtd' in OPS/section-0002.html
ERROR: IOException: Unresolved external XML entity: 'http://www.w3.org/TR/xhtml11/DTD/xhtml11.dtd' in OPS/section-0003.html
ERROR: Unable to locate element with id 'tableofcontents' in OPS/toc.ncx
or
The table of contents contains one or more links without text
even though the file passes Epubcheck.
The problem is that Lulu can't cope with bookmarks in the NavMap (toc.ncx file). The solution is to use a NavMap that doesn't have bookmarks, and you can achieve this by checking the configuration option Force simple NavMap. This overrides whatever NavMap you might have created using the command Format | Book | Build Table of Contents, and generates a simple, one-entry-per-section NavMap with no bookmarks. Your table of contents page is not affected by this.
Force simple NavMap won't work if Jutoh is not generating the NavMap itself - if you are editing NavMap entries manually, or have imported them from an existing Epub file, you may need to edit them and remove the bookmarks manually. You can edit your NavMap by clicking on Edit on the toolbar; click on Indexes, and then the Contents page. Click on each entry and remove the hash character and following text, if any.
If a hierarchical logical table of contents (NavMap) is important to you, consider using an alternative platforms such as Draft2Digital. Bear in mind, though, that some ebook readers will not show the full NavMap hierarchy anyway.
